I am asked in visa application form to fill in Surname and given name. It also says If only one name is in passport enter it as Surname. And at the given name box it says "If your passport does not include a given name enter FNU". But my passport has no surname, and only given name. So what am I supposed to do?

Ted Pack’s instructions are correct. However, using you father’s name as a surname could create some problems because of your Visa not matching your passport. If you wanted to use your father’s name as a surname, you should have your passport changed to match. As an alternative to your father’s name, many Indian’s use their caste as their surname.
